If you are a renter, an Owner Authorization Form is required prior to service being transferred into your name. The form is available here in PDF or as a Word Document. • Physical address of the service location
• Billing address (if different)
• Social security number or driver’s license number
• Employer contact information (if applicable)
• Customer phone number (home, mobile, work)
• Signature(s)
• Payment of fees, according to Fee and Rate Schedules.
